AES-LRW document for voting

The attached doc incorporates final edits for the LRW mode. I would like to submit it for voting at the Oct 29 meeting.

1) All references to AES key sizes removed.

2) Made consistent with Dalit's Key Backup "Key Scope" definitions and references these

3) References to 512 byte units removed except by way of examples

4) "May" and "Shall" clauses made consistent with IEEE usage.
