SCC20 Membership—You must complete this form and submit or return it no later than December 23, 2011 if you wish to be a member of SCC20 during 2012. This applies to current as well as new members. Failure to complete and return this form will cause your name to be deleted from the IEEE SCC20 mailing list. You will be classified as an "Interested Party" during 2012 if your form is received after the above date. You may also select “Interested Party” to remain informed of meetings and activities.

All entries except those marked with a * are required entries. Failure to provide required entries may result in your submittal being ignored.

Note that to be classified as a Member you have to be an active participant in at least one subcommittee. The most obvious evidence of active participation is to have attended at least one sub-committee meeting. Sub-committee membership allows access to working documents of the subcommittee.
