 Device, not adapter. Adapter (power supply) could be only power source.

 Adapter has nothing with it - it render only (some) devices more complicated. Phone would be probably only sink, but laptop could be sink and source (for e.g. phone).

 Medical devices - also devices compatible with IEEE 11073-30200 series, some of them (almost everyone of them) are to be at home.
 Medical restrictions are not meant "to be harder", but "more safely"; as we want power supply to last many years, it could be somewhat more expensive.
