The text in the proposed changes to the bylaws mandates the use of the IEEE Standards Board approved form.  If these changes are approved, that would be the LOA form.

The reason is that if the form is not used, someone must thoroughly examine whatever is submitted to insure it is in full compliance with our bylaws and operations manual.  That someone would be the IEEE's legal counsel.  That is typically an iterative process that takes a lot of time and costs a lot of money (which could probably be spent more effectively elsewhere.)

I would point out that more and more SDOs are moving to requiring a specific form.  That includes ISO, IEC, ITU, TIA and others.

To the IEEE Pat Com:
 
    This is my first message / question to this group.  I apologize, in advance, if you covered this before (extensively).  Would you please tell me if a firm wants to declare IP, is the use of the Letter of Assurance optional?  
    One relevant section of the By Laws (6.2) states "If the IEEE receives notice that a [Proposed] IEEE Standard may require the use of a potential Essential Patent Claim, the IEEE shall request licensing assurance, on the IEEE Standards Board approved Letter of Assurance form, from the patent holder or patent applicant. The IEEE shall request this assurance without coercion."
